Care Within Reach partners with hospitals, physician offices, case managers, and social workers across Georgia to ensure a seamless transition from facility to home for medically fragile children. As a licensed GAPP provider, we accept referrals for children 21 under who require skilled nursing or personal care services in the home. Our clinical team works directly with referring providers to develop and implement individualized care plans that meet each child’s unique medical needs.
